In 2020, when the Covid-19 pandemic began, the Professor Alejandro Posadas National Hospital implemented clinical monitoring of patients and their contacts by telephone to manage the overwhelming demand for assistance and avoid overcrowding. This led to the formation of a Telephone Monitoring team. The team's success prompted the hospital to explore other areas where the tool could be applied. In 2021, the hospital continued to use the Telephone Monitoring team, adapting it to new lines of work where patient care traceability was critical for ensuring access to the healthcare system. This article examines the role of patient navigation in improving healthcare outcomes, including better access and quality of care for underserved populations. The hospital institutionalized the Telephone Monitoring device in February 2023 as a sector of Assistance Relinking, adding another weapon to benefit patients by increasing their safety and improving the quality of care.

RESUMO

Abstract Background and objectives: It has been observed a general public increased search on the Internet for health information, including Anesthesiology. The objective of this study was to evaluate the information available to the lay person in Portuguese on the Internet about labor analgesia for the Brazilian population. Method: Using the term "labor anesthesia", the first 20 sites found on Google in November 2014 were evaluated by two resident physicians and classified as medical and non-medical. Legibility and Design - accessibility, reliability and navigability-were compared using Flesch Reading Ease Score (FRESH) and Minervation validation tool for healthcare websites (LIDA) tools. The websites' content was confronted with that of the medical literature. Results: Medical and non-medical websites were considered difficult to read according to FRESH. Regarding the design, there was no difference between groups regarding navigability, however, accessibility was considered superior in non-medical websites (p = 0.042); while reliability was higher in medical websites (p = 0.019). Conclusions: With the increased search for health information on the Internet and concern about improving the quality of childbirth care, it is fundamental that the content available to the layperson about labor analgesia is of quality and well understood. This study demonstrated that both medical and non-medical websites are difficult to read and that non-medical websites are more accessible while the medical ones are more accurate.

A variety of structured assessment tools for use in surgical training have been reported, but extant assessment tools often employ paper-based rating forms. Digital assessment forms for evaluating surgical skills could potentially offer advantages over paper-based forms, especially in complex assessment situations. In this paper, we report on the development of cross-platform digital assessment forms for use with multiple raters in order to facilitate the automatic processing of surgical skills assessments that include structured ratings. The FileMaker 13 platform was used to create a database containing the digital assessment forms, because this software has cross-platform functionality on both desktop computers and handheld devices. The database is hosted online, and the rating forms can therefore also be accessed through most modern web browsers. Cross-platform digital assessment forms were developed for the rating of surgical skills. The database platform used in this study was reasonably priced, intuitive for the user, and flexible. The forms have been provided online as free downloads that may serve as the basis for further development or as inspiration for future efforts. In conclusion, digital assessment forms can be used for the structured rating of surgical skills and have the potential to be especially useful in complex assessment situations with multiple raters, repeated assessments in various times and locations, and situations requiring substantial subsequent data processing or complex score calculations.

RESUMO

OBJECTIVE: Investigate the quality of data available on the internet with respect to pain after orthognathic surgery. METHODS: A careful search was conducted on the Internet in December, 2012. The most accessed websites browsers were employed for research using the terms: "pain" and "orthognathic surgery" together. The first 30 results of each portal were examined, and after applying the exclusion criteria, 29 sites remained. All remaining websites went through an evaluation process with online tools that investigated the quality, level of reading, accessibility, usability and reliability. RESULTS: Assessment criteria outcomes were considered unfavorable. Texts were considered difficult to read with inappropriate language for the general public. The mean global validation for the 29 websites of the LIDA instrument was 65.10, thereby indicating a structure of medium quality. CONCLUSION: Information about post-orthognathic surgery pain available on the internet is poorly written and unreliable. Therefore, candidates for orthognathic surgery must seek information from specialists who, in turn, should indicate reliable sources. .

RESUMO

BACKGROUND: The majority of patients undergoing stem cell transplantation (SCT) require a blood transfusion until the complete engraftment. Because blood transfusion rules for patients with ABO-incompatible SCT are complicated, we developed an ABO-incompatible transfusion management system (ABO-ITMS) for accurate blood transfusion and improved manageability. METHODS: A committee composed of medical doctors, technicians, and a programmer developed ABO-ITMS during the eight months from July 2013 to February 2014. The program has been linked with other databases, including clinical and laboratory databases and resulted in a new subsystem of the health information system. Server computer's operating system was Window Server 2008, and the database manager program was Oracle 11g. Programming language was ASP.Net (VBScript, C #), and the server and client computer were used to connect to the web server using a web browser. RESULTS: ABO-ITMS was designed to follow three main steps by hematologic oncology clinic, laboratory physician, and blood bank. In the first step, a hematologic-oncology clinic doctor inputs SCT recipients' data and appropriate ABO group for each phase of post-transplantation. Laboratory physician enters the isoagglutinin titer and ABO group at the second step. Finally, blood bank workers enter the results of type, screening, and antibody identification. The patient's SCT information and the previous immunohematologic test results are shown on the screen. CONCLUSION: ABO-ITMS can replace the existing complicated system and workflow. ABO-ITMS will contribute to reducing medical error and improving quality of SCT recipient care.

RESUMO

OBJECTIVES: The number of healthcare institutions adopting smartphones continues to increase, implying that their utilization is undoubtedly gaining attention. Understanding the needs of smartphone users will provide a greater opportunity for successful information technology acceptance by expanding the scope of its utilization. This study focuses on how smartphones are accepted and utilized in hospitals and analyzes the factors influencing users' attitude, social influence, and intention of use. METHODS: For the study model, the researcher has mainly adopted the Theory of Reasoned Action and further modified and used the models of Technology Acceptance and Information Systems Success. To test the model empirically, a survey was conducted with 122 professionals on information development teams in Korean tertiary hospitals. RESULTS: The common smartphone usage modes were Internet searching, e-mail, scheduling, and social networking in consecutive order. Phone calls consisted of 51.4% of work-related purposes, while other functions, such as text message, Web browser, and scheduling, were mostly used for personal purposes. Costs, contents quality, innovation, ease of use, and support were shown to have statistically significant effects on user attitude, and social influence, portability, security, content quality, and innovation were significant. User attitude and social influence were both statistically significant with respect to intention of use, with user attitude greater than social influence. CONCLUSIONS: The participating staff were analyzed as having strong personal faith and principles, independent from their external environment. Timely information exchanges among medical staff will facilitate appropriate communication and improved health services to patients in need.

RESUMO

OBJECTIVE: We developed a Clinical Document Architecture(CDA) Generator module based on CDA standard for the interchange of radiological reports. METHODS: This paper describes CDA standard, the template of radiological report, CDA Generator module, and the Web-form report using an Extensible Stylesheet Language(XSL) style-sheet. And the CDA Generator module is integrated into a existing Picture Archiving Communication System(PACS) Viewer. RESULTS: Radiological reports based on CDA standard are used to interchange between different health institutions, and also presented in an Extensible Markup Language (XML) compatible web browser. CONCLUSION: The proposed module and concept in this paper may be a utility in improving health care delivery and can also be used to integrate with other Digital Imaging and Communication in Medicine(DICOM) Structured Report (SR) compliant PACS systems.

RESUMO

PURPOSE: The authors have developed a system that searches for radiologic reports, and we have evaluated the utility of this system for clinical application, research and medical education. MATERIALS AND METHODS: The system retrieves radiologic reports from the OCS (ordering communication system) server and stores them in a separate computer. A CGI (common gateway interface) program was written to search the database with variable keywords given by the user. The system satisfied the requirements of functionality, accessibility and security. RESULTS: Users could search for radiologic reports with variable keywords using the web browser, and the user could refine the results using combinations of these keywords. The system has been running successfully for 2 years, and radiologists have used this system for searching or collecting cases, keeping reports and gathering statistics. CONCLUSION: The simple text searching system has been developed in a short time period with only small costs and a bit of hard effort, and this system is practical for clinical use, research and medical education.

RESUMO

Embedded computing and wireless communication technology were used to develop a mobile biotelemetry system, featuring the functions of mobile measurement, wireless transmission, database archival, and internet retrieval of biomedical signals. Two PPCs (Pocket PC) with WLAN (wireless LAN) PC cards were used to simulate the communicating situation of biomedical signals between wireless sensors and a personal monitor. A PPC client program simulated the wireless sensors and a PPC server program simulated the personal medical monitor. Medical PDA was implemented by SBC (single board computer), LCD display, touch screen and Bluetooth module. Windows CE.net was ported as its operating system and the application program was developed using embedded Visual C++ 4. The measured transmission speed of ECG data was 2150 samples per second in the WLAN environment. For wireless internet connection, a Web-based server program was developed, able to display the biomedical signals of remote patients and be remotely monitored by medical doctors in a distant hospital. PDA data, which was archived at the server's database table, was transformed to an XML message and could be displayed on a Web browser as a signal graph. This system would be very useful for patients with chronic illness, especially in an emergent environment, and for containing medical cost in the aged society.

RESUMO

The amount of medical information is rapidly increasing in accordance with the development of medical technology. Therefore, developing efficient ways of management, exchange and sharing of medical information is becoming an important issue. A variety of models and tools introduced in hospitals, departments and clinics became obstacles in the reproduction and utilization of information. A proficient means of exchanging this medical information, known as an Application Program Interface (API) has been designed in the present study. A discharge summary which contains major description of medical care has been chosen as the source for this exchange. The API translated the information on the discharge summary into Health Level Seven (HL7) messages, which is the standard protocol for exchanging information. Users could send and/or receive a discharge summary using a web browser using this API. For the security of a web-server, we used a freeware, Mr.Flux, to control access to the server. Time to input data was significantly different according to the traffic of network, which was 341+/-55 seconds and 131+/-15 seconds at 10 p.m at 10 a.m. In summary, HL7 based on an API for the exchange of information on nuclear medical care could be used in different computing environments such as an internet based system that is convenient to use for most users.
